For those already living in the community, or transitioning from supported living, our outreach services provide the support they need to build and manage their lives. We actively involve people we support and their families in their care planning, to ensure the approach suits them. 

By assisting the people we support to set achievable goals we support them to develop new skills and continue to live fulfilling, independent lives.
