Outreach services

For those already living in the community, or transitioning from supported living, our outreach services provide the support they need to build and manage a life they have chosen. We actively involve people we support and their families in their care planning, to ensure the approach suits them.

By assisting the people we support to set achievable goals we support them to develop new skills and continue to live fulfilling, independent lives.

Supported living

Our homes are currently a mix of self-contained flats and shared houses. As with all our supported living services, our support teams work together closely with developers to provide a quality, suitable home and support plan tailored to each person, to help them ultimately live independently and as part of a local community.

We pride ourselves in being able to facilitate not only suitable homes for the people we support but desirable homes, with every comfort possible to allow the people we support to live as independently as possible.

Day services

Our day centres offer a person-centred approach, supporting people to develop skills, find the hobbies they love, and meet their life goals.

For some of the people we support the day centre is their base for the day, where they meet friends, enjoy social and leisure activities and access educational opportunities. For others the day centre enables them to develop new skills and access new opportunities.

We work closely with individuals to be proactively involved in their care and support plans enabling them to work towards their goals and achieve positive outcomes.

John has a really good team working with him now, thank you. James is a very calm influence on John, and Jane - what can I say, she’s just amazing with him
